In a the 12 contiguous leads of an ECG, the septal leads are V1 and V2. Thus, with a septal wall myocardial infarction, we see ST-segment changes and elevation in leads V1 and V2. Septal infarcts stem from insult to the left anterior-descending coronary artery (LAD).

The left anterior descending coronary artery, or LAD, is also termed the "widow maker," because it supplies 45-55% of the left ventricle. Occlusion of this artery can lead to septal wall infarct.
