Sodium bicarbonate is commonly used in TCA toxicity. Two mechanisms are postulated. First, alkalinization of the plasma can increase protein binding of TCAs and decrease their bioavailability. Second, the sodium load is thought to reverse the Na+ channel blocking effects of TCA.
