Teriparatide works to increase osteoblastic activity, which leads to bone growth. This effect occurs when administered in a pulsatile fashion.

Teriparatide has a short duration of action and half-life, and, as such, the initial effect is bone resorption. This effect is then followed by an increase in bone formation. Thus, when administered in a pulsatile fashion, this stimulates bone formation compared to resorption and, over time, leads to increased bone density.

As a PTH analog, teriparatide is commonly used to treat osteoporosis. It is also approved for the treatment of glucocorticoid-induced osteoporosis and can be considered a first line in higher-risk patients.

Teriparatide should be used with caution in those with elevated risk factors for osteosarcoma. For instance, those with increased unexplained ALK-P, Paget disease of the bone, or those with a history of cancer or radiation therapy.

Teriparatide use can result in transitory hypercalcemia.
